Wellness Program

The City and County of Broomfield offer a Wellness Program to provide employees with a framework to help them live healthy, happy and productive lives at work and at home.

Participation in the Wellness Program also helps to reduce out-of-pocket costs for health insurance for benefited employees enrolled in the Medical Care Expense Plan (MCEP) administered by UMR - United Healthcare Choice Plus Network. For benefited employees not enrolled in the MCEP, you will receive additional annual leave for participating.

Broomfield Healthy Rewards program is customized just for you!  

If you are in the Medical Care Expense Plan (MCEP) you have the ability to earn, for the next plan year, the premium discount by completing an online Clinical Health Risk Assessment (CHRA), and Biometrics.
Let's Get Into Being Well 2018 - UMR

If you are enrolled in the Kaiser Medical plan, you have the ability to earn 8 hours of annual leave, by completing your Health Assessment in your Kaiser portal. 

If you have waived your coverage of both Kaiser and MCEP, you have the ability to earn 8 hours of annual leave, by completing a Clinical Health Risk Assessment (CHRA).

Biometric Screening Form - Take this form with you to your doctor's appointment to have completed at your well-check appointment.
Medical Exception Form - This form should be completed if the 5 out of 5 biometric outcome-based goals are not met.
Broomfield Healthy Rewards Google Form - This form should be completed once you have your biometric screening form finished. Please DO NOT send your biometric forms directly to HR.

We are introducing this year "Transformational Wellness" in partnership with Health Promotion Management Inc.   This program will include quarterly Classes and Challenges and monthly email check-ins.

Recreation Facilities ...........................................................................................

Employees are eligible to use the recreation facilities at no cost. Please refer to the EMPLOYEES’ USE OF CITY AND COUNTY RECREATION FACILITIES POLICY for guidelines on this benefit.

Take a copy of your last pay stub and $5.00 to either the Broomfield Community Center (BCC) or Paul Derda Recreation Center (PDRC) to have your employee pass activated.
